If you would like a credit application, display the Credit Application page and print it. We can also fax you a copy or you may pick one up at any of the stores. Please fill it out, sign it and fax it to us at (413) 781-3512 or mail it to HZ Electric Supply, P.O. Box 1605, Springfield, MA 01101-1605. The form is in PDF format and requires the free Adobe Acrobat Reader to display.

Customers are required to pay sales tax unless we have the appropriate forms on file that show they are an exempt organization, a reseller of goods they buy from us, or a contractor doing work for an exempt organization. Check out our Sales Tax page for the appropriate forms.
